The gostek's hitbox is divided into multiple parts — head, torso, legs — and each can have a different damage multiplier. Intuitively, headshots cause the most damage, while hits to the leg hurt less.
Depending on your weapon and the current game's weapon mod, this can mean a difference of multiple hits needed to kill the enemy. A close range Spas-12 shot, with all pellets hitting the head, can eliminate the enemy in one click. And while Dragunov is able to kill with two hits, more than two are needed if you're only shooting the enemy in the leg.
